Nurse Practitioner – Outpatient Only - Value Base Care
Location: Jacksonville, FL
Monday–Friday | 8:00 AM – 5:00 PM

Manage and coordinate care for patients with chronic conditions

Florida license
Knowledge of HEDIS and HCC coding
